ePermits online applications (HTML): This link takes you to the TCEQ's epermits online Notice of Intent, No Exposure Certification and Notice of Termination application forms to get instant authorization or to instantly terminate a permit.

Notice of Intent (PDF): This form is required to be submitted for facilities which discharge storm water associated with industrial activity. The instructions detailing how to fill the Notice of Intent are included in this form.

Conditional No Exposure Exclusion Certification (PDF): This form is required for those facilities that can certify that there is no discharge of storm water associated with industrial activity from a regulated facility.

Notice of Change (PDF): This form is required to be submitted for making updates, changes, or corrections to an NOI or Waiver as required in the general permit.

Notice of Termination (PDF): This form is required to be submitted for regulated facilities when there is a change in operator or owner or if the facility closes (and there is no longer a discharge of storm water associated with industrial activity). The instructions detailing how to fill the Notice of Termination are included in this form.

Industrial Sectors Subject to Permitting Requirements (PDF): This document details which Standard Industrial Classification (SIC) codes and Industrial Activity (IA) codes are subject to the TPDES Multi-Sector General Permit No. TXR050000.

Hazardous Metals Waiver Form (PDF): This form is required to be completed and kept on site at regulated facilities that can certify that the hazardous metals listed on this waiver meets at least one of the three criteria found in Part III.D.1.(e) (i-iii) for the discharge location/respective outfall that is authorized by the TPDES Multi-Sector General Permit.

Benchmark Monitoring Form (PDF): This form is required to be submitted for Semi-annual benchmark sampling, which must be conducted at least once during the first full monitoring period (January through June or July through December) following permit issuance, and then once during each monitoring period for the term of the general permit.
